03Mechanism of action
Cagrilintide
Cagrilintide is a long-acting analogue of amylin, a pancreatic hormone that induces satiety. It is studied alone and combined with semaglutide (CagriSema).
Retatrutide
Retatrutide (LY3437943) is a single peptide agonist of three receptors: GIP, GLP-1 and glucagon. Adding glucagon-receptor activity is studied for increasing energy expenditure and liver-fat effects on top of the appetite effects of GIP/GLP-1 agonism.
04Study endpoints
Cagrilintide
- Phase 2 (26 weeks): every cagrilintide arm reduced weight more than placebo (6.0%–10.8% vs 3.0%); the top arm also beat liraglutide (10.8% vs 9.0%).
- REDEFINE 1 (3,417 adults without diabetes): cagrilintide–semaglutide gave −20.4% mean weight change at 68 weeks vs −3.0% with placebo.
- REDEFINE 2 (type 2 diabetes): −13.7% vs −3.4% with placebo at 68 weeks.
Retatrutide
- Phase 2 obesity trial (338 adults): mean weight change at 48 weeks ranged from −8.7% to −24.2% across arms vs −2.1% with placebo.
- In the highest arm, 83% of participants lost 15% or more of body weight at 48 weeks, vs 2% on placebo.
- Preclinical and early clinical work describes glycaemic and weight effects from combined triple-receptor agonism.
05Safety & regulatory findings
Cagrilintide
- Investigational: not approved as a standalone medicine.
- Phase 2: gastrointestinal events in 41%–63% vs 32% on placebo, mainly nausea. REDEFINE 1: gastrointestinal events in 79.6% vs 39.9%.
Retatrutide
- Investigational: not approved by the FDA or EMA for any indication. Phase 3 trials are ongoing.
- Phase 2 adverse events were mainly gastrointestinal and dose-related; heart-rate increases were also reported.
