03Mechanism of action
PT-141
PT-141 (bremelanotide) is a melanocortin receptor agonist, acting mainly on central MC4 receptors involved in sexual motivation.
Melanotan II
Melanotan II is a non-selective melanocortin receptor agonist. MC1 activity increases melanin (tanning); central melanocortin activity produces erections and sexual stimulation.
04Study endpoints
PT-141
- RECONNECT (two phase 3 trials, 1,267 premenopausal women with hypoactive sexual desire disorder): significant increases in desire and reductions in related distress vs placebo.
- Integrated safety data (1,247 women): nausea was the main reason for stopping treatment.
Melanotan II
- An early phase 1 study in healthy men reported increased facial and body pigmentation, spontaneous erections, nausea, and a stretching and yawning complex.
- A qualitative study of 623 online forum entries found use driven by tanning goals, frequent misinformation and concerning practices with unregulated products.
05Safety & regulatory findings
PT-141
- FDA-approved (as Vyleesi) only for hypoactive sexual desire disorder in premenopausal women; research-grade material is not that product.
- Most common adverse events: nausea (40.0% vs 1.3%), flushing (20.3% vs 1.3%) and headache (11.3% vs 1.9%). Small, transient blood-pressure increases were seen.
- Focal hyperpigmentation was rare at labelled use but affected more than a third of participants after up to 16 consecutive daily administrations.
Melanotan II
- Not approved by the FDA or EMA for any indication; research-grade material is not a medicine.
- Case reports link unregulated use to rhabdomyolysis, kidney failure and renal infarction.
