Vasoactive intestinal peptide (VIP; aviptadil as a drug) is a 28-amino-acid neuropeptide acting on VPAC1/VPAC2 receptors, causing vasodilation and anti-inflammatory effects in lung and gut.

• The earlier critical-COVID trial missed its primary endpoint despite a reported secondary survival signal. The subsequent TESICO placebo-controlled trial did not demonstrate significant clinical benefit from intravenous aviptadil and was stopped for futility; neither formulation validates supplier VIP vials.
• Inhaled VIP in pulmonary hypertension: a modest, short-lived drop in pulmonary vascular resistance without lowering systemic blood pressure.

• Not approved by the FDA or EMA for any indication; research-grade material is not a medicine.
• Safety observations concern the specific trial formulations and populations, not supplier VIP vials; the later TESICO efficacy result must be considered alongside earlier reports.
