01

Mechanism of action

NAD+ is a coenzyme central to energy metabolism and a substrate for sirtuins and PARP enzymes. Most human trials raise NAD+ indirectly with precursors such as nicotinamide riboside (NR); trials of injected NAD+ itself are scarce.

02

Observed research endpoints

  • NADPARK (30 people with early Parkinson's, 30 days): oral NR raised brain NAD levels, changed cerebral metabolism, lowered inflammatory cytokines and was associated with mild clinical improvement.
  • Pilot RCT in 20 older adults with mild cognitive impairment (10 weeks): blood NAD+ rose 2.6-fold with NR; cognition scores stayed stable.

03

Regulatory & safety context

  • Precursors such as NR are sold as dietary supplements; injectable NAD+ is not an FDA-approved drug.
  • Both trials reported NR as well tolerated, with no between-group difference in adverse events in the cognitive-impairment trial. These findings are for NR, not injected NAD+.
