03Mechanism of action
MOTS-c
MOTS-c is a 16-amino-acid peptide encoded in the mitochondrial 12S rRNA gene. In mice it acts mainly on skeletal muscle, inhibiting the folate cycle and purine synthesis, which activates AMPK; under metabolic stress it moves to the nucleus and regulates gene expression.
NAD+
NAD+ is a coenzyme central to energy metabolism and a substrate for sirtuins and PARP enzymes. Most human trials raise NAD+ indirectly with precursors such as nicotinamide riboside (NR); trials of injected NAD+ itself are scarce.
04Study endpoints
MOTS-c
- In mice, MOTS-c treatment prevented age-dependent and high-fat-diet-induced insulin resistance and diet-induced obesity.
- Plasma levels of MOTS-c decline with age, according to a 2023 review.
NAD+
- NADPARK (30 people with early Parkinson's, 30 days) tested oral NR, not NAD+ vials. Brain NAD increases were variable; altered cerebral metabolism and mild clinical improvement were reported in NR recipients whose brain NAD increased. Inflammatory cytokines also decreased.
- Pilot RCT in 20 older adults with mild cognitive impairment (10 weeks): blood NAD+ rose 2.6-fold with NR; cognition scores stayed stable.
05Safety & regulatory findings
MOTS-c
- Not approved by the FDA or EMA for any indication; research-grade material is not a medicine.
- A 2023 review notes no effective clinical method of using MOTS-c has been developed; evidence is preclinical.
NAD+
- Precursors such as NR are sold as dietary supplements; injectable NAD+ is not an FDA-approved drug.
- Both trials reported NR as well tolerated, with no between-group difference in adverse events in the cognitive-impairment trial. These findings are for NR, not injected NAD+.
