SS-31 (elamipretide) is a mitochondria-targeted tetrapeptide that binds cardiolipin in the inner mitochondrial membrane and is studied for stabilising mitochondrial energy production.

• MMPOWER-3 (218 adults with primary mitochondrial myopathy, 24 weeks): no improvement vs placebo in 6-minute walk distance or fatigue.
• An earlier crossover trial missed its primary endpoint but showed signals in patient-reported outcomes, which supported MMPOWER-3.

• The FDA granted accelerated approval to elamipretide as Forzinity for a specific Barth-syndrome indication in September 2025. That approval does not establish benefit for primary mitochondrial myopathy or approve supplier research vials.
• Generally well tolerated in the cited myopathy trials; injection-site reactions were the most common event (80% in the crossover trial), mostly mild.
